- FORM: Notification of a change to corporation directors, contact person or secretary
Corporations must notify ORIC of changes to their officers’ details within 28 days of the change. - FORM: Request to change corporation name
Corporations must notify ORIC of a change to their corporation's name within 28 days of the special resolution being passed. - FORM: Request to change corporation rule book
To request a change to their rule book, corporations must lodge the request within 28 days of the special resolution being passed. - FORM: Notification of a change to corporation address and/or contact details
Use this form for notifying ORIC of changes to the corporation's address and/or contact details. - FORM: Notification of appointment or resignation of auditor
Corporations must notify ORIC within 14 days of an appointment or resignation of an auditor. - FORM: Application for reinstatement
Use this form to apply to reinstate a corporation that has been deregistered. - FORM: Voluntary deregistration
Use this form to apply to deregister a corporation.
Exemption forms
- 1400 FORM: Application for exemption (general)
- 1401 FORM: Application for exemption from having a minimum of five members
- 1403 FORM: Application for exemption in relation to members
- 1404 FORM: Application for exemption or extension of time in relation to meetings
- 1405 FORM: Application for exemption in relation to member approval for related party benefits
- 1406 FORM: Application for exemption in relation to directors
- 1407 FORM: Application for exemption from record keeping
- 1409 FORM: Application for exemption from reporting requirements
